Enrollment

• Submit a Preschool application. This form can be completed by contacting the Preschool Office at (859) 744-4637 or may be downloaded below. English and Spanish version are available. Please print and complete the application and return it to the Learning Resource Center at 100 Vaught Road, Winchester, KY 40391
   

• ·Gather enrollment information, including child’s social security card, birth certificate, immunization certificate, & income verification forms (such as income tax returns, pay stubs, Medicaid card, benefits information, etc.).
   

• · After application has been submitted, you will be contacted to schedule an appointment for screening.  The screening process helps determine  whether your child meets income eligibility requirements or may need an evaluation to determine if a delay or disability exists.  If screening results reveal a potential area of delay, the school system will offer to schedule and conduct an individualized evaluation (at no cost to you) to determine the nature of the delay or disability.  Examples of delays & disabilities include:  speech or language delays, developmental delays (in areas of communication, motor, cognitive, social/emotional, and/or self-help skills), certain medical or health impairments, and more.  After the evaluation has been completed, a meeting will be held with you to determine if your child has a delay or disability that entitles him/her to receive preschool services.
   

•        To complete the enrollment process, your child will also be required to have a physical exam by  his/her family physician and a vision exam prior to school entry.  Please be sure to tell your physician & optometrist to document exam results on Preschool Entry forms.  Return these forms to the Preschool Office.
   

•        Once your child has met all eligibility requirements, you will be notified of your child’s acceptance into the Preschool Early Education Program.  Placement decisions for school locations may be made at a later date, but every effort will be made to place your child at the elementary school within the district of residence.
